Estrogen does increase the risk for breast cancer. It's not a very big increase but it is a significant increase and it looks like the longer that a woman takes estrogen, the greater that her risk will be of breast cancer. I think that's an issue that a woman needs to discuss with her doctor. But if her quality of life is so adversely affected, and if her long-term life expectancy is great, then we consider giving her estrogen replacement, even though breast cancer is estrogen sensitive. People that have liver disease, bad liver disease have problems taking estrogen. However, people with diabetes and heart disease absolutely should take estrogen because those are the women that are highest risk for cardiovascular disease. Also, if she's not taking... The companion to estrogen, the companion hormone called progesterone, she's at a higher risk for developing endometrial cancer, which means cancer of the uterus. So for women who have a uterus in, we normally would prescribe progesteron as well as estrogen. A woman's general health is also very important and is affected by estrogen. The organs of the bladder, the urethra, which is the tube which drains the bladder to the outside, and the vagina are sensitive to estrogen. And when estrogen is deprived after the ovaries stop making it. The vagina starts to thin out in atrophy, the bladder starts to thin out, and the urethra starts to think out. In terms of the urinary system, that can lead to problems with a condition called stress incontinence. When a woman coughs or sneezes, she may lose her urine because the bladder isn't strong enough to keep the urine in. A woman's sexual health is significantly affected by the loss of estrogen because she can't make vaginal secretions very well, which leads to very painful intercourse. And also the vagina can start to thin out enough so that even without intercourse, it becomes very painful. And these conditions can all be helped by estrogen, even in very low doses.
text/plain
https://uoregon.aviaryplatform.com/collections/1635/collection_resources/71407/file/157360#t=832.32,951.66
https://uoregon.aviaryplatform.com/collections/1635/collection_resources/71407/file/157360/transcript/89973/annotation/43
Speaker 22: The aides will come over about two or three hours in the morning and two or three hours evening and all the rest of the time we got to ourselves.
text/plain
https://uoregon.aviaryplatform.com/collections/1635/collection_resources/71407/file/157360#t=992.66,998.66
https://uoregon.aviaryplatform.com/collections/1635/collection_resources/71407/file/157360/transcript/89973/annotation/44
Speaker 23: They're free to come and go as they please.
